Errrh... オカマ (okama) has the meaning of "effeminate, flamboyant, most probably gay". "Twink" designates a gay man with a thin and youthful appearance and is not a fitting translation in this case. Dude looks like a mix of the Laughing Buddha and Mr. Clean ;)

I am not sure there is a single-word equivalent to オカマ in this context. You'd have to ask somebody gayer. Pretty good job otherwise!

@Boxen closest term that comes to mind as a direct translation, but it's an adjective more than a type of person.
