The gallery on the creator's Pixiv appears to skip from page 4 to 6 and I can't find any signs of a page 5. His tumblr only appears to have pages 1 and 8, and his Deviantart has broken links to several pages (including the missing page 5) that presumably were deleted due to lewd.
