It has to do with two things. 1) The complete "fall" of the love interest where they're "willing" to fuck anyone or anything beside the MC. This is for those who enjoy the corruptive aspect of NTR because it exhibits how much the love interest has fallen morally. 2) The utter humiliation and isolation of the MC, where everyone except them get to have a "good time". I like your explanations of some of the common elements of NTR. its a fucking shame that i dont particularly like either ending. also you forgot to mention that obviously they cant A: make the MC win in the end or B: have the NTR guy suffer for his actions because that tampers with the fantasy. you this isnt a genre really conducive to such endings 99% of the time as people are associate themselves with either the sadistic NTR guy or the cuck MC (in a masochistic manner) and having the roles be reversed or their arcs upended isnt something people are particularly interested in and even if it were to occur it will most likely be the MC or 'good guys' falling from a higher standing to a lower one as the story progresses.

NTR isnt a place for happy ending most 'happy' endings are usually rife with the looming threat of inevitability whether its the big dick old man dying as a result of poor health leaving the girl without the very thing she forsook everything for or the marriage of the girl to the 'friend' quickly devolving into domestic disputes because she didnt marry him to be in a relationship with the guy shes there for dick and as soon as she gets bored or he stops preforming at the level that stole her away she just needs to "turn a trick" and have someone pick her up.... it just how NTR works
