Its futa, you can see how she has no balls and the penis is coming out of her vagina. The dialogue also supports this, that dick was grown through the power of science. As for why she looks so different on the back cover, that would be because thats not her, thats a different character. She has no chest in the comic pages because she has a flat chest. Very boyish character.
