I feel like there is some psychological meaning that could be gleaned about the author of this doujin. I assume the author feels himself in the role of Kaworu in this story. His internal conflict is represented by the multiple (alternate universe) versions of the character. His real self (the one from our universe) needs to be tied up and held down, too afraid to ever indulge in his taboo feelings he has for Shinji. I get the sense that this represents the way the author feels out in the real world. If general society knew about his taboos, he fears he would be shunned by those around him. That's where the alternate versions come in. They represent that part of himself that he wishes could force himself to except his taboos and be the person he knows he is inside. It's a liberating experience that the author wishes he could experience in reality, but instead uses his art to express his hidden desires.
