The ending is left very ambiguous when it comes to the final episode of 5Ds. This is due to everyone going their separate ways in order to achieve their individual dreams, with Yusei staying in New Domino and Akiza going to Germany to study as a doctor.

It's very possible they had a kiss when the camera panned away but did not officially get together because the long-distance would make it difficult to maintain a relationship. I also remember hearing about Yusei's Japanese voice actor believing that the two do end up together. So, if the voice actor's words are true, then it's likely Yusei and Akiza get together after the 8-year time-skip, when everyone has largely reached their dreams and are coming back for a reunion.

Can you only imagine the sexy times those two could have, especially with Akiza having an even more mature body and full control over her psychic powers?!
