At this point I assume the artist doesn't know what he/she want to create: futa or otokonoko. The previous cg with same theme have otokonoko in the title (this gallery have both "futanari sensei" and "otokonoko") when the "girls" have boobs, girl body structure and penis made them closer to shemale instead

Well, the title could be interpreted that the beautiful doctor who is assumed as 'futanari' by the protagonist turns out to be "otokonoko" or femboy (shemale or newhalf since the teacher got a pair of nice boobs).
