I saw there was no scan of this available anywhere, and the book is going for $100+ on eBay, so I got it from a local library and scanned it. 200 DPI is more than enough for this, some cropping and correction.
There are a few two-page spreads I chose not to compile, they can be better-viewed in a pdf version.
