Well first of all you're reading into it a little bit too much. Even IRL quite a lot of people enjoy seeing other's misfortune or comeuppance — fiction just allows you to further experience situations which you otherwise couldn't for various reasons, and this extends to other kinds of feelings too. NTR is unique in porn because it's the only genre that by default deliberately tries to evoke an emotional response other than horny.

As for liking it, I've seen a lot of reasons. Some self-insert as the bull, some enjoy the particular brand of female degradation/corruption that's usually involved, some literally just have a cuckold fetish, some are just here to jerk off and can't read. Personally I find NTR best when the author actually writes a proper fucking story because they realise it's a sub-genre of a larger one: tragedy. Yes really I'm not joking. Our "heroes" are the MC and FMC who initially have a hopeful future but fail to reach it due to their flaws, which begins a downward spiral into self-destruction. Knowing what could've been, the reader should experience a deep sense of sadness or pity (pathos). Then once rock-bottom is near, the MCs confront their past and realise they have come to ruin. They've made the wrong choices and lost what is most important to them. This part is most important because knowing the "full ending" so to speak gives the reader that particular emotional release, or catharsis.

So getting back to my original comment, what I really want is catharsis. This doujin was actually setup half decently for it! We have the hopeful beginning of MC and FMC getting together in the future, we get the slow downfall of MC realising he lost his chance to confess and FMC still feeling regret for giving into the brother's advances, but just when it's ripe for that sweet disastrous confrontation.... it ends. To complete the tragedy you'd probably want the MC to confess his feelings and idk, become a NEET, suffer permanent anxiety, self-harm or something, while the FMC continues her relationship out of obligation but never feels happy. Just an example, the more fucked up the better really. You want to finish the story and feel like someone truly, truly fucked up.

tl;dr: I think good NTR should be like a Greek tragedy and have a proper cathartic ending. Writing 101 shit.

A girl can like a guy, but if he does nothing she'd be taken away by a guy she doesn't like as much but who's more proactive. Even in modern society girls rarely take initiative, unless it's a drunk desperate fat girl who's looking for casual sex you'd better not get your hopes high.
