What's to be confused about? It's a brothel where you buy a (non-refundable) girl to fuck before you kill her in order to use her as meat. The male protagonist ejaculates enough to inflate the meatgirl's belly, then she hands him a knife to slit her throat while she squeezes out his enormous load into a bucket, but he prefers to impale her alive instead. Presumably this is fairly normal behavior in this world, since the proprietor of the brothel does not take issue with his treatment of their livestock at the end of the visit and invites him back. It all seems very straightforward to me.
