If I had to guess, I think Mizuha developed the app because she's the honor student. And all of them had previous versions since their childhood but when it got out of control they agreed to never use it again and hypnotize each other into forgetting about the app and distancing from each other. Except Mei. The only one still close to Umeka she probably pretended to forget about the app (or someone else like Mizuka or Marina hypnotized Mei) and then Mei hypnotized Umeka into discovering the app again and hypnotizing the other 2. Mei is probably currently not pretending though and she is under the app of someone that the other 2 have, or she could even be under the control of both Mizuha and Marina, that could be what the fatal error is about.
