For every other npc, a barnacle is a one-hit-kill, since if alive they would have to deal with completely new sets of animations and physics.
For Gordon, though? No fatal neck snapping, just a lift up and you get bitten to death if you have poor aim. If you die, you don't even get swallowed like a typical ragdoll, since you are still using a first-person camera. You just die and fall back down.

It's not a difficult fetish to understand even if you're not into it. Going inside a wet dripping hole, the sexual connections are obvious for dudes, and for girls (or faggots) it has that vulnerability and giving up control aspect.
