Dont get me wrong it has some hot scenes, but 5 fucking years for this?
Just not enough "After" content. Most of it is scenarios that take place during the main game routes, with 1 maybe 2 scenes continuing the ending scenes of each route. Results in scenes that are too similar to the original.

Consider the original yasui route. We were left off with a very corrupted Ryouka, tattooed and taking drugs. You'd think we would get a deep dive into how this happened and the escalation of that. Witnessing her dye her hair, getting a tattoo, but no. Instead it just gives us more prostitution scenes that feel like filler and they skip to the content we actually want to see right at the end for 1-2 scenes only.

I was baffled when the author had said that he took a break during development because the NTR broke him, and now Im even more confused. With the exception of the Nobuo ending, the actual NTR factor, involving Keita, is very light. I really wanted to see Ryouka be corrupted and become incredibly filthy past the point of no return, but that aspect is just lacking here. She is just so passive throughout every route.
We could've been 5 years into Tiina 2 development instead.

Are there any new routes in this game?
Or is it the same game but with some extra scenes thrown in? I feel like the routes that were in the original game already felt complete enough.

All the images above are new scenes. It's basically an extension off of each guy's route. So the homeless guy has an extra 7 or 8 scenes, the black guy has an extra 7 or 8 scenes, etc. At the beginning of the game, you pick the guy you want extended scenes for, and then it plays their route. One of the guys are locked in the beginning of the game and you have to pick another route first. Then you can pick their route. And the last bonus needs all the other routes played first.
