Well, actually I have a crazy idea about the true story of this doujinshi.
Probably, the huge black face guy is a god of the shrine(the one the boy and the girl went at the beginning).

In page30 and page31, there is a flashback, which indicates that the girl had made a vow to the god of the shrine. Her wishes were "not put the boy in danger" and "have the boy's child". As we can see, she did put her boyfriend in danger, but she also has his child. So, maybe the god came to grant her wish, but also to punish her / let her learn how hard her boyfriend try to protect her.

I've also considered that maybe the black face guy is not a god but simply a gang leader sent by God, but the boy's aside in page8&9 ("is he even human?!" "monstrous speed and strength...") and page 34("It feel like we were played by a ghost though") just hints me. Maybe the black face guy is so strong and disappeared after that case because he is an incarnation of god.

Another reason I believe he's a god is the last page of this doujinshi. The after story in page34&35 is so sweet and cute, and I don't think the author wants to draw a dark and sarcastic ending like "the girl actually have the child of the gang leader". So, I would rather think positively that the black face baby means the baby born with the god's blessing.

I know it sounds crazy, as the gang leader looks very much like a bad guy and seldom do author portray a god in Japanese as black, dark face. Still, I think it might be a good guess.
