It can be somewhat misleading, but to clear it up: the narrator's point of view correspond to the main character, and he can't see what's going in pages 21 and 22, so his imagination fills the blanks. Page 24, and the final panel with the word 'VIP' in bold confirms it beyond reasonable doubt.

Well, Gunma's stories are all top notch in my opinion, but the twist in this one was certainly enjoyable.
