It's one of the BLUE series of games. (The most famous is Triangle Blue, which was also adapted to an h-anime). These games tend to start off with the MC and the main girl as a couple. I would argue True Blue is the most famous and it is the game that started the series. Its true that the first version of Triangle Blue came out first but that version was just your typical harem fickle cheating protagonist douchebag game; it was due to the success of True Blue that they went back and made an NTR version of Triangle, which is great cuz that douchebag deserves to be NTRed hard.

Its quite funny how many of these NTR games actually has the guy being more of a traitorous doucebag than the girl, though I suppose that is normal for fap-based H-games in general.

There are a few pretty boys also trying to NTR her so I can see why one might be confused on whether the pretty boy is the MC. The MC doesn't really have any face, sprite, or design but he should have a few H scenes with her. I haven't played the game so I can't really say but they usually do.
