Here's an explanation:

Humans make robots. Robots have errors. Humans begin fixing errors. Humans get enslaved and raped. Errors persist.

The errors in the process might be intentional of the machines .... errors will reproduction process will cause errors in the humans it makes... errors in humans being made means less chance of an uprising against machines......

The explanation for the continued errors is simple. The robots are robots, not A.I.s Someone programmed the robots to follow a set of procedures. Ready the women as directed. Dispose of one's that are non viable. There are errors in the programs and procedures that sometimes make the women defective. But the robots are robots, they don't learn or care about errors, mistakes or productivity. The robots will simply follow their programming for as long as they function. So the women have even less hope than if they were ruled by artificial intelligence. The robots don't care. The robots will never care. They will just continue to operate their flawed human breeding factory. Impassively modifying women's bodies and destroying their sanity, uncaring of their suffering no matter how much they beg, scream or cry.
