Ok, from what I got Satou got into an accident where he got badly injured when he discoverd his girldfriend Rin having sex in their apartment with some man.
Said man with glassess is the director of the place where Rin works part time, he lusted after her and have his way on her very easy because she had debts and needed the money, never told Sato about it and he(the director) offered her the money if he slept with him so the she became his cumdumpster, she had "no option" left but obey him.
He also told Satou too bad he survived his accident, he would be better dead.
She(Rin) is a lost cause she already is on the typical "Sorry *insert your name here*, I can't defy his words and my body only obeys him" bullshit mode.
At the last page of chapter 3 her boss decided to keep banging the wife in front of Satou while the nurse(Okashira?) was like "My god, this is just too much cruel".

Protagonist gets visit by the milf from chapter 2(she is the one who send the MC to the hospital with her car and found no better way to apologize other than having sex with him) who wants to service him but doesn't knows why he looks so down and can seem to get his friend down there up so she leaves worried.
The nurse notices she left and as days passes by mc's health worsens by a lot and says he is fine like that, dying in that accident would have been better so he will choose to die now.
The nurse feels heart broken and as she was close to him she kind of felt empathy she heals with with love and sex but rejects him at the end when he asks her to be his girlfriend.

Chapter 7
As you can see the nurse's fiance has her as his plaything but in the hospital behaves like a saint.
Since her marriage is arranged, she endures it, and MC has no say in their relationship, however he won't allow him to abuse her anymore(she doesn't likes him at all but is tied to the arranged marriage).

Chapter 8
After a talk between MC and the nurse while the milf and childhood friend they hear about doctor Hasegawa's true colors.
Well, the doctor Hasegawa is very salty thathis fiancé rejected him and now in order to take revenge from the MC he lied the milf and his tomboy childhood friend that they needed medical check.
He kidnapped, tied and drugged both to rape them. First he rapes the milf while the childhood friend is watching tied, after he ends chapter ends with him saying it's the other one turn.

It might look like he has the advantage for now but he is doing it wrong, using, drugs, lotions and toys while the MC didn't need any of them despite being hospitalized.
