Well, I think that's a big part of the appeal, honestly.

Frankly there are tons of doujin out there that depict stuff that's far more vile, in completely realistic (real world) settings. Making it about elves and orcs separates it, for me at least, enough away from reality that the terrible degeneracy of it is arousing, instead of upsetting.
