Interestingly, the author respects a lot of canon/official stuff:
- contrary to what some might believe, Shiki likes Azaka and dislikes Touko,
- Azaka is physically stronger than Shiki (see 6th movie),
- Azaka is naturally attracted to forbidden things, hence her incestuous love for Mikiya.

Mix those up and this doujin isn't really all that far-fetched IMO.
