Interesting read.
I'm a bit lost on what Mari means when she tells Kengo that Saki doesn't know anything about Mika, or what Mika means at the very end about feeling weak and pathetic.
Is the implication just that Mika is struggling with being a guy (partly for Saki's sake) and Saki doesn't really see that?

(All of these are my own opinions and how I interpreted the story)
To answer your first question, I see it as how Saki clearly doesn't give anything back to Mika in that she never tries to help Mika or learn about him / her.
As for the ending lines, it seems to me that it's a similar case in that Mika gives everything to Saki and yet perhaps has self doubt as to whether or not they are helping Saki.
I personally see it as a sense of powerlessness that despite giving everything to Saki, Mika feels defeated and pathetic in not being able to solve all of Saki's problems. And so, I think that Mika feels equally disgusted at themselves for knowingly choosing this path (when they could've chose surgery to go back). And clearly Mika still has a lot of unease and fear in terms of the future.

Also, I forgot to mention this in my TL afterword but, in case you didn't pick up on it, it seems that Saki's first time was with that cousin that showed up briefly near the end.
