I can't read Japanese, so maybe someone fluent in the language can give me more context and insight on the story and plot as I can only go off of the picture until an English translation gets made.
But I believe this story started out as any typical NTR story does. The childhood friend has feelings for the heroine, but he hasn't confessed either because he's in denial and wants to keep seeing her as a "boyish" friend. The heroine is well aware of her childhood friend's feelings, but either thinks he's too sexually immature for her, has no sexual interest in him at all or doesn't want to hurt his feelings and ruin their friendship. She also has a boyfriend (or sexual partner) that satisfies her sexual needs and can't let her childhood friend know this. She seems considerate enough of her childhood friend's feelings as she was willing to hold back as much as she could with the sex between her and her secret boyfriend while talking to her childhood friend from across their respective windows. She even stepped on her partner's penis to hold him off as best as she could when he got too aggressive and it could risk their secret.

She's very comfortable seeing her childhood friend as just a reliable friend to hang out with. She's very platonic towards her childhood friend even when teasing him with sexual innuendoes like when the melted ice cream fell on breasts or when she used her feet to mess with him under the table. This seems to cross the line when her childhood friend took her feet, while she appeared to be asleep, and used them to pleasure himself. She appears to be doing less sexual activities with other guys and has focused more on her boyfriend. She invited her childhood friend over and they spend some time together. It looks very innocent, but it appears her childhood friend still has feelings for her judging by the "heart" that his speech bubble has. After they eat, they have another awkward moment with seeing each other naked. She teased her childhood friend before they finish their (presumably separate) baths. Her childhood friend is about to go home but before her childhood friend leaves, she tossed her shirt at him and teases him with her bare body. This seems to be a last goodbye between them. Some time has passed and the heroine has moved in with her boyfriend and they have a lot of fun together, growing closer. I don't know if she ever told her childhood friend that she was moving away, but this seems to have been motivated by her wanting to keep distance between her and her childhood friend for both her sake and maybe his own sake. I like this heroine. She isn't written to be heartless unlike other NTR heroines. I also see influences of Casca from Berserk in her design. I want to keep liking her and this story, but the NTR gives me the same agony that she felt between her childhood friend and her boyfriend.

Ameno is doujin artist whose content centers around foot fetish more than anything else. I like foot fetish doujin. I don't like NTR. This particular work seems to be the only one of Ameno's work that has heavy NTR in it. This is also Ameno's "biggest" work. The well drawn feet and hot sex scenes sell this work alone. The NTR isn't really needed to sell the foot content. Whenever NTR gets introduced, it usually overtakes the entire doujin because it's one of the plot-based fetishes or themes. It's upsetting, depressing and distracting to have NTR hanging over an otherwise sexy, foot fetish doujin. You can basically do any other plot that isn't NTR nor as unpleasant as NTR. I hope this heroine moving away to be with her boyfriend symbolizes a meta-narrative change to moving away from NTR as a story element. I get people want something "spicy" or "messed up" because they got to hit that next sexual high and the "cheating" or "rape" tag isn't cutting it anymore for them. But it ruins the fetish that guys like me are originally here for. A lot of artist that add NTR to their stories, usually end up ONLY focusing on the NTR, or they stop doing updates altogether. NTR is narrative crutch and is toxic for story arcs because the story will never progress from the initial downfall of character relationships. The story becomes stuck in a loop with NTR until the artist decides to end it. And because of this, the ending is usually abrupt, jarring and upsetting. I'm saying all of this because I am passionate about the works of artist that make art of my fetishes. I hope Ameno continues this story, but on a sexier AND brighter path. I am hoping.
