For the gender bender, the guy turns into a girl when he has sex with a female, and a male when he has sex with a male. At the end he must have overdone it because he ends up as a okama/hermaphrodite.

What aoi said is mostly true. Except in the very last dialogue box the character says after half a year they finally fully became a girl.
