Story/Spoilers:
The old man is the protagonist's father. Because the protagonist isnt able to impregnate his wife (medicinal issue), the father forces him to make a choice:
Let him, the father, have sex with her to get an heir or get kicked out of the house.
Like expected, the pussy protagonist agrees and after a while the wife, too.
The first time you play through this game, you see only the shadows (his wife and his father having sex behind japanese doors) from the perspective of the protagonist (252-262).
After the first and second (the wife is still not pregnant) "love-making", the player is confronted by a desperate wife and can make the only choice in the game:

a)Leave the house with her(why are you playing this game) or
b) stay, let the Dad continue "child-making".

The first choice just ends the game (a more or less happy ending for protagonist and his wife),
the second results in the wife enjoying slowly the sex and finally cheating on her husband (68-99).

73-81: The wife telling hero she's going for groceries with his dad. Dad having fun.

248-251: the protagonist wakes up, realizing his wife is missing. Hearing noises in his father's room, he opens the door, watching them having fun. The wife doesn't know he is behind her (the father does, just grinning at him :3) and he gets to listen her screaming how she freakin loves his father's dick.

Conclusion: B+ Good, short netorare. Love the style of team ntr, playing with the imaginations of the player because he doesn't see anything, just shadows and sounds.
Just later you unlock the scenes from the wife's perspective.
